 Failure to Follow Suit Situation 

West led a diamond won in dummy.
Declaring South cashed 3 top spades, LHO somehow not seeing the J and discarding a diamond.
Declarer, perhaps surprised at the inference of West's not following suit, surmised that East began with the balance of the spades, four in total, and made some sound of displeasure at that variance, major or not, perhaps a memory aid:
"Four card major for a 2 preempt, eh???"
and called for a 2nd heart discard from dummy.

East, with really only 3 spades, might have wondered of the question, holding only 3.
Was the statement actionable?
It probably wasn't 'disinformation', but it was a suggestion to West of East's hand shape and a false suggestion, obviously a case of a declarer that wasn't counting right and gave ready evidence of it, at least now to East.

West then wakens from impaired spade vision, or properly unfolded hand, and found the J, placed it on the table and summoned the director. "There's been an irregularity."
The penalty card was established.
Declarer, previously upset with the four card major variance, was upset anew that East would have taken perhaps a full six seconds to play third and last spade.
"How could you take so long to play your spade when you have only one to play???"
The director gave declarer option to retract dummy's play and later suggested that singletons, even if part of 3 card suits, should be played without pause for thought, perhaps the "4 card major" recounting, or miscounting, or misinformation, actionable or not, not having been related as possible 'just cause' for pause.

There may or may not have been suggestions of impropriety, shadyness or otherwise, in taking so long to play the lone at the time East third spade and last spade.
There didn't seem to be a lot of sympathy at the table just then.
Perhaps there was some East and South history.

After the game, East wondered of declaring South's allusion to possible impropriety even after the 'four card major, eh???' comment:
"Tells my partner a false story of my preempting with a side four-card-major, and I'm supposed to play in tempo, eh???.
